LTC ACO is the first Accountable Care Organization to serve Medicare beneficiaries who live in long-stay facilities. Since 2016, we have focused on this high needs population which includes the frail elderly, individuals living with Alzheimer’s disease or dementia, depression, amputations, developmental or physical disabilities, renal failure and those with multiple chronic diseases and complications. 

LTC ACO is a pioneer in leveraging the benefits of an accountable care organization in the long-term care industry and is deeply committed to the success of its participating partner practices. By participating in LTC ACO, primary care clinicians and long-stay facilities are empowered by an expert team committed to helping them advance the state of healthcare for residents in their care.

We empower long-term care clinicians and facilities to deliver value-based care by analyzing and providing actionable data and insights that drive improved quality and cost performance. Additionally, unlike other managed Medicare programs, such as Medicare Advantage Institutional Special Needs Plans (I-SNPs), LTC ACO partner practices have NO downside risk and NO capital contribution is needed to participate.

MEET OUR LEADERSHIP

Al Shainepresident

Al Shaine, MBA, serves as President of LTC ACO, the Medicare Shared Savings Program dedicated exclusively to improving the coordination and quality of health care provided to Medicare beneficiaries residing in long-term care and assisted living facilities throughout the United States. Mr. Shaine also serves as the President of AlignMed Partners, a founding member and current participant in LTC ACO. AlignMed Partners collaborates with clinical and operational leaders to enhance clinical capabilities in skilled nursing and long-term care settings.

Prior to joining AlignMed Partners as COO in 2019, Al served as a Regional Vice President for Privia Health, where he managed operations across the Mid-Atlantic region and implemented strategies to drive ACO performance. Before Privia, Al served as a Senior Director with The Advisory Board Company, where he advised health systems’ in-network development strategy and revenue cycle performance improvement.

Al holds an MBA from Georgetown University’s McDonough School of Business and a Bachelor's degree from Wesleyan University in Middletown, CT.

Kristen KrzyzewskiChief Strategy & Program Development Officer

In her role, Ms. Krzyzewski developed the initial concept for LTC ACO and obtained CMS approval of the initial and renewal applications. She works closely with the organization’s CEO to evaluate and design strategic initiatives that support the long-term growth and success of LTC ACO. Prior to joining LTC ACO, she served as the senior vice president, corporate planning and development for ValueOptions, a managed care behavioral health company, where she led the organization’s dual eligible and Medicaid Aged, Blind and Disabled strategy. She previously served as senior vice president, business development for HealthSpring, a Cigna company, and Bravo Health/Elder Health where she was responsible for new market development and service area expansions, including state licensure and CMS approval; development of I-SNP, D-SNP, and Medicaid proposals (TX STAR+PLUS); and strategic planning and market/competitive research. Ms. Krzyzewski earned a master’s degree in business administration from Harvard Business School and a bachelor of arts degree in economics from Cornell University.

Maria C. SuarezChief Compliance Officer

Ms. Suarez joined LTC ACO and Genesis HealthCare as chief compliance officer in June 2022. She has almost 30 years of risk management, contracting and compliance experience in  executive compliance roles overseeing corporate compliance programs for health care and long-term care entities. Ms. Suarez previously served as chief compliance officer for Regency Integrated Health Services in Texas, where she oversaw the corporate compliance program for all of Regency's long-term care facilities. A deeply experienced legal and compliance professional. Ms. Suarez is certified in Health Care Compliance (HCC) from the Health Care Compliance Association (HCCA). She is a University of Miami School of Law graduate, a licensed attorney in Florida, and a member of The Florida Bar since 1990. 

Laura Slack, MD, CMDMedical director

Dr. Slack serves as the medical director for LTC ACO. Dr. Slack brings a wealth of experience to LTC ACO, having practiced as a board-certified geriatrician in long-term care for over 15 years and served as regional medical director for many states. She is active in AMDA/PALTC with a special passion for medication management, palliative care and wound care. She attended college at the University of North Carolina Wilmington, where she graduated with honors. While pursuing her medical degree, Dr. Slack focused on elderly patients during medical electives and mission work in China, the United Kingdom, West Indies, and West Africa. Dr. Slack completed her family practice residency and geriatric fellowship at Florida Hospital South and Florida Hospital Winter Park respectively. 

Walter Lin, MD, MBAClinical Strategy Officer

As the Clinical Strategy Officer for LTC ACO, Dr. Lin focuses on the development of the ACO’s clinical strategy to support improved clinical outcomes for the ACO and its participating groups. He is the Founder and CEO of Generation Clinical Partners (GCP), a medical practice focused exclusively on caring for those in senior living organizations. Dr. Lin has first-hand experience with numerous value based programs, including ACOs, Institutional Special Needs Plans (I-SNPs), Bundled Payments for Care Improvement (BPCI), Program of All-Inclusive Care for the Elderly (PACE), and home-based medical care for the seriously ill, and serves on the Physician-Focused Payment Model Technical Advisory Committee (PTAC) for the Department of Health and Human Services. Prior to founding GCP, Dr. Lin worked as a healthcare venture capitalist for Ascension Ventures and led Clinical Innovations for Catholic Healthcare West. Dr. Lin trained in internal medicine at Yale and earned his MD from the University of California, San Francisco, MBA from the University of Pennsylvania's Wharton School of Business, Masters of Science from the University of California, Berkeley, and undergraduate degree from Stanford University.
